Please fill in the following information and list the dates that would be best for you and your organization. Our schedule is filling quickly, so please list alternate dates in the event that we are booked on a date that you choose. We will make every effort to accommodate you.

In order to reasonably assure that the sale will be successful, we require that you take a few moments and provide the following information:

1.*Number of personnel at your location?

2.*What type of business are you?

3.*How many women at your location?

4.*Are you located in a multi-tenant building? Yes No

If so, will you be able to advertise the sale with each tenant? Yes No

5.*Would we be able to set up sale in the lobby, cafeteria or area of highest visibility or traffic? Yes No

6.*Would you be able to supply a minimum of three (3) six-foot tables and one (1) electrical outlet?(We can accomodate sales of all sizes) Yes No

7.*Does your organization
offer Payroll Deduction? Yes No
